A Checklist for Submitting a Transportation Quote

  • Full insured name
  • Both mailing and garaging addresses
  • DOT and MC numbers
  • Brief submission narrative including lines of coverage needed, target pricing and how we can help you write the account
  • Limits and coverages desired.  Auto Liability, physical damage, cargo, GL etc.
  • Number of years the insured has been in business.
  • Prior carrier and loss history for all lines of coverage requested.  Loss runs, if available.
  • Drivers List including name, date of birth, driver’s license number, state, number of years CDL experience, violation/accident history for the past three years.  MVRs are great if you have them but not required.
  • Full Unit Information, which includes year, make, model, VIN, actual cash value, whether the vehicle is owned, leased, or an owner/operator.
  • Radius the insured operates on.  Include farthest states and any major cities frequented.  IFTAs if available.
  • Percentage breakdown of commodities being transported.
  • Disclose any certificate or contract requirements.
